Open System Sciences (OSS) offers 3 group medical plans with CareFirst BlueCross BlueShield Insurance Company. BluePreferred, which provides Preferred Provider Organization (PPO) coverage and BlueChoice, which provides Health Maintenance Organization (HMO) coverage, and it also provides HMO Opt Out Plus Health Plan. The rates for PPO are as follows; individual insurance coverage is provided at a cost of $164.50 per month. Family coverage is at a cost of $460.50 per month. The rates for the HMO are as follows; individual insurance coverage is provided at a cost of $97.50 per month. Family coverage is at a cost of $272.50 per month. The rates for the HMO Opt Out options are as follows; individual insurance coverage is provided at a cost of $124.50 per month. Family coverage is at a cost of $348.50 per month. OSS's dental plan is with Delta Dental. Individual insurance coverage is provided at a cost of $5.40 per month. Family coverage is at a cost of $18.01 per month. The company also sponsors Short-Term/Long-Term Disability, Accidental Death and Dismemberment and Life Insurance (maximum of $100,000.00).

The OSS 125 Premium-Only-Plan provides employees electing medical coverage the opportunity to reduce costs by using pre-tax dollars to pay for health and/or dental premiums.

401(k) PLAN

OSS offers employees the opportunity to contribute up to a maximum of $13,000.00 of their salary
annually to the 401(k) plan on a pre-tax basis. All employees are eligible from date of hire with two entry dates (January 1 and July 1). A number of investment options are available to employees.

PROFIT SHARING PLAN

OSS offers a profit sharing plan, which is entirely company supported with all contributions made by OSS. All full time regular employees are eligible to participate in the OSS profit sharing plan. An employee becomes eligible after 1 year of service.

EDUCATION AND TRAINING

OSS' Education Assistance Program assists employees in furthering their education and career development. Upon approval, tuition/training reimbursement is available. For more information, see OSS handbook or contact Human Resources.

COMPUTER PURCHASE PROGRAM

In order to assist employees in obtaining personal computer equipment for their own use, OSS will provide employees with the opportunity to request an interest free loan to be repaid through payroll deductions within a twelve-month period. Employees are eligible to participate in this program after completing their introductory period with the company. See Human Resources for eligibility and other details.

RECOGNITION AND AWARDS PROGRAM

OSS' Recognition and Awards Program was established for the recognition of employees for outstanding performance and consists of the Employee of the Quarter and Employee of the Year award. This also includes a Tenure Recognition program, awarding those employees at their 5th, 10th, and 15th anniversary.

OSS' Commuter Choice Tax Free benefit was created to assist those employees who use mass-transit in their daily commute. By participating in this benefit, employees can have their monthly commuting expenses deducted directly from their paycheck, pre-tax.

VACATION BENEFITS

OSS' full-time regular employees accrue paid time off based on their length of service:
" 10 days per year for the first five years of employment (accrued at 6.66 hours per month)
" 15 days per year for employees with 5-10 years of service (accrued at 10 hours per month)
" 20 days per year for employees with more than 10 years of service (accrued at 13.32 hours per month)

OSS offers ten paid holidays per year. The holiday schedule is published on an annual basis.
